Removing an Incident Investigation that a worker started

A worker started an Investigation but the incident does not warrant an investigation. How can a HSE Supervisor or Site Supervisor delete it?

Please note: Once the Incident report is “Finalized”, it cannot be deleted from the system, nor can any investigation that is created as a result of the finalized incident report.

Incident Reports can be deleted when it is in the Open or Pending Assignment stage. This gives the opportunity to decide if the report warrants an investigation.

  1. From your dashboard, click on Investigations
  2. Find your Incident Report under the heading “Investigations Pending Your Review” or “Your Open Incident Reports”
  3. On the right side, click  Trash Can on the report to be deleted.
  4. You will have one more pop-up asking “Are you sure?” Click Yes if the incident does not warrant an investigation
